How Patient-Centered Care Reduces Readmissions Rates and Reducing Readmissions Improves HCAHPS

The patient experience paradox: no matter how wonderful the experience – patients hope to be healthy enough to avoid the experience again. No one really wants to be in the hospital. No one dreams of being a patient. This gets to the heart of most new payment models and financial incentives. The broad purpose is to reduce excess care and move care delivery to the lowest cost setting – generally outside the hospital.
